Job Overview
We are seeking a compassionate and experienced Family Nurse Practitioner to join our multidisciplinary care team. In this role, you will provide advanced primary and acute care, promoting health and wellness through patient-centered treatment and preventive strategies.
Key Responsibilities
- Perform comprehensive health assessments and physical exams
- Diagnose and manage acute and chronic conditions
- Order, interpret, and act on diagnostic tests (labs, imaging, etc.)
- Prescribe medications and therapeutic interventions
- Develop, implement, and monitor individualized patient care plans
- Educate patients on disease prevention, health maintenance, and treatment adherence
- Document patient encounters accurately in EHR
- Collaborate with physicians, specialists, and care teams for coordinated treatment
- Maintain compliance with HIPAA and state/federal regulations
- Participate in ongoing professional development and quality improvement initiatives
Required Qualifications
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from accredited institution
- Active Nurse Practitioner license in California
- National certification (AANP or ANCC as Family NP)
- Prescriptive authority and DEA registration
- 2+ years of clinical experience in primary care or family practice
- Proficiency in EHR systems and diagnostic tools
- Current BLS/CPR certification
Preferred Qualifications- Experience working in outpatient, clinic, or hybrid settings
- Proficiency in electronic health records and telehealth platforms
- Strong communication, critical thinking, and patient advocacy skills
- Bilingual (English/Spanish) is a plus
